Rally’s, 3492 S. U.S. 41 (2 Critical, 2 Non-Critical) — Found bucket of sanitizer water stored above boxes of food; no certified food handler on staff.

McDonald’s, 3032 Wabash Ave. (2 Critical, 2 Non-Critical) — Found raw eggs stored over package burritos in small upright cooler; brown residue found in front handwash sink.

Burger King No. 13396, 1160 U.S. 40 (2 Critical, 1 Non-Critical) — Ice tea nozzle at drivethrough found with debris; soda nozzles in lobby found with black debris; lettuce chopper put away unclean; pass through pans, trays and metal racks found with greasy buildup.

Moose Lodge, 3708 Wabash Ave. (2 Critical, 1 Non-Critical) — Meatballs in cooler found past date of consumption (discarded); pink debris found on ice machine chute.

Wendy’s, 2803 Wabash Ave. (1 Critical, 5 Non-Critical) — Lobby and drive through nozzles found with debris.

Uncle Herbie’s, 2206 N. 25th St. (1 Critical, 4 Non-Critical) — Raw pork cutlets and eggs stored above ready-to-eat foods.

Holiday Inn, 3300 S. U.S. 41 (1 Critical, 2 Non-Critical) — Bar soda nozzle holders found with debris.
