RESTAURANT INSPECTIONS: July 2, 2012

TERRE HAUTE — June 11-15:

McDonald’s, 2633 S. Indiana 46, (2 Critical, 3 Non-Critical) Food residue observed in handwash sink; Flies observed throughout dining room and kitchen.

Los Tres Caminos, 5147 S. U.S. 41, (2 Critical, 2 Non-Critical) No one in charge at beginning of inspection; Certified food handler expired.

The Smoothie Hut Café & Oasis, 3401 S. U.S. 41 H.C. Mall, (2 Critical, 2 Non-Critical) Observed ice and chocolate syrup in handwash sink; Found debris on ice machine chute.

Taj-Mahal, 1349 S. Third St., (2 Critical, 2 Non-Critical) Soda fountain nozzles found with accumulated black debris and veggie dicer found with old food debris; Spray bottles not marked with common name of chemical.

TGI Friday’s, 3401 S. U.S. 41, (2 Critical, 1 Non-Critical) Dried food observed on knives in rack, Automatic dishwasher not dispensing sanitizer.

Thornton’s #80, 2330 S. Third St., (2 Critical) Ice chutes on soda fountain and the spout to the Banana freeze drink were found with accumulation of debris; Milk in the milk dispenser was observed at 51 degrees, should be 41 degrees or below.

The Olive Garden Italian Restaurant #229, 3820 S. U.S. 41, (2 Critical) Wait station soda fountain nozzles found with black/brown debris; Tongs hanging on wall in prep area found with burnt on black debris.

Dew Drop Inn, 1805 N. 19th St., (1 Critical, 4 Non-Critical) Bar soda nozzle found with brown debris.

Beef “O” Brady’s Family Sports Pub, 2309 S. Third St., (1 Critical, 3 Non-Critical) Potato salad and shredded cheese in topping cooler found at 53 degrees, should be 41 degrees or below.

Texas Roadhouse Holdings LLC, 2941 S. Third St., (1 Critical, 2 Non-Critical) Employee observed rinsing pot in handwash sink.

Maurizio’s, 2940 Wabash Ave., (1 Critical, 2 Non-Critical) Handles on handwash sink observed with debris.

Qdoba Mexican Grill, 3510 Riverside Plaza Drive, (1 Critical, 1 Non-Critical) Salsa verde was found with date mark of 6/3/12 which is 2 days past the date of which to use.

Subway, 1120 Wabash Ave., (1 Critical, 1 Non-Critical) Mold observed on chute in ice machine.

Kentucky Fried Chicken #0221, 4400 S. U.S. 41, (1 Critical, 1 Non-Critical) Deboned chicken in walk-in cooler held past date to discard.

Ryves Hall Youth Center, 1356 Locust St., (1 Critical) Found box of rotten tomatoes in walk-in cooler (discarded).

Sav-A-Lot, 3 Plaza North Shopping Center, (1 Non-Critical)

The Landing at Fort Harrison, 3350 N. Fourth St., (1 Non-Critical)

Bethany House, 1402 Locust St., (1 Non-Critical)
