RESTAURANT INSPECTIONS: May 28, 2012

TERRE HAUTE — May 7-11

Jade Garden, 3375 S. Third Place (6 Critical, 7 Non-Critical). Beaten eggs found at 76 degrees — should be 41 degrees or less; debris found on bar nozzle and inside nozzle holder; rice on baking sheet pan found at 98 degrees — should be 135 degrees or above (discarded); observed an employee scoop cooked food out of pan onto plate with bare hands (food discarded); found dirty knives stored with clean knives in bucket, observed roaches in kitchen by wok and stove, mouse feces in fryer cabinet. Voluntarily closed until reinspection May 9.

Imperial Lanes, 400 N. Third St. (3 Critical, 4 Non-Critical). Front and back bar soda nozzles and soda nozzle holders found with accumulated debris; small toaster oven rack found with built-up debris; snack bar handwash sink found dirty.

Antique Dairy Queen, 395 Davis Drive (1 Critical, 3 Non-Critical). Handwash sink found dirty.

Sweet Lou’s Pizzeria Express, 2619 Fort Harrison Road (1 Critical) Pizza screens found with burnt food debris.

Dollar General 4504, 940 Poplar St. (1 Critical) Found several dented cans.

Rick’s Smokehouse & Grill, 3100 Wabash Ave. (2 Non-Critical)
