TERRE HAUTE — Holiday Inn, 3300 U.S. 41 S. (7 Critical, 6 Non-Critical) Employee observed loading soiled dishes and unloading clean ones without washing hands; strawberries stored in drawer with raw meats in Apple Club Grill area; sausage gravy 123 degrees, potatoes 103 degrees in warmer - should be 135 degrees or above; cooked rice pilaf in Apple Club Grill not dated; crème brulée, potatoes, eggs, milk and juice held past date to discard; employee observed dumping coffee into handwash sink; fruit flies observed throughout kitchen, Apple Club Grill and outer bar area.

La Tienda De Maria, 5127 S. U.S. 41 (3 Critical, 7 Non-Critical) Spoiled avocado and produce observed in refrigerators, frozen bacon and chorizo held past expiration date; raw beef stored above carrots in refrigerator, pork chorizo stored on top of cheese in reach-in cooler; beef in steam table found at 125 degrees - should be 135 degrees or more (discarded). Establishment closed until re-inspection scheduled for May 18.*

Fraternal Order of Eagles, 823 Poplar St. (3 Critical, 3 Non-Critical) Tongs observed in handwash sink; raw hamburger stored over pickles, cheese and sandwich toppings in refrigerator; mold observed on soda fountain nozzle.

Real Hacienda, 2750 S. U.S. 41 (2 Critical, 3 Non-Critical) Spray bottles of cleaners not labeled; table mounted can opener and knife in rack observed with food debris.

Outback Steakhouse, 3700 S. U.S. 41 (2 Critical) Accumulated debris found on stainless steel baking sheets; sweet potatoes and baked potatoes found in warmer drawer left from the night before (discarded).

Papa John’s Pizza, 4842 U.S. 41 S. (1 Critical; 4 Non-Critical) No proof of certified food handler. Follow-up inspection scheduled for May 16.*

Becky’s Café, 1462 Lafayette Ave. (1 Critical, 1 Non-Critical) Chicken and noodles not cooled within appropriate time limits.
