RESTAURANT INSPECTIONS: Dec. 17, 2012

TERRE HAUTE — The Vigo County Health Department inspected the following food establishments Dec. 3- 7:

Smoothie Hut Café & Oasis, 3401 S. U.S. 41 (5 Critical, 2 Non-Critical) — No one left in charge with food safety knowledge; frozen beverage mixes found on cart at 52 degrees (discarded); cooked gravy temped at 110 degrees after several hours (discarded); chili, gravy, hard-boiled eggs, sausage and egg not date marked; debris observed on ice chute inside ice machine.

McDonald’s, 2633 S. Indiana 46 (2 Critical, 2 Non-Critical) — Drive-through soda nozzle found with debris; pass-through wire racks found with accumulated grease and debris.

Jiffy Mini Mart No. 582, 1560 N. 25th St. (1 Critical, 5 Non-Critical) — Ice chute and base of soda nozzles observed with an accumulation of debris.

Long John Silvers, 2021 Wabash Ave. (1 Critical, 2 Non-Critical) — No sanitizer measured in wiping cloth bucket and in three-bay sink.

Magic Wok, 3401 S. U.S. 41 (1 Critical, 2 Non-Critical) — No one in charge at time of inspection.

Uncle Al’s Pecans Inc., 1616 Poplar St. (1 Critical, 2 Non-Critical) — Chili, veggie soup, tuna salad and cole slaw held past date marking.

Hulman Links 19th Hole, 990 N. Chamberlain Road (1 Critical, 1 Non-Critical) — Black debris found on soda fountain nozzles.

Texas Roadhouse, 2941 S. Third St. (1 Critical, 1 Non-Critical) — Observed employee filling a plastic container with water in handwash sink.

Fort Harrison Mini Mart, 1301 Fort Harrison Road (1 Critical, 1 Non-Critical) — Ice machine chute observed with mold.

Sparks Stop 5 Tavern, 1915-17 Lee Ave. (1 Critical, 1 Non-Critical) — Debris observed in soda nozzle cup holder.

Comfort Suites, 501 E. Margaret Ave. (1 Critical, 1 Non-Critical) — Sausage links and scrambled eggs temped at 122 degrees and 130 degrees respectively; should be 135 degrees or higher.

Clarion, 3053 U.S. 41 S. (1 Critical, 1 Non-Critical) — No sanitizer provided.
