McDonald’s — 2111 Lafayette Ave. (3 Critical, 1 Non-Critical) Canadian bacon found at 82 degrees  —  should be 41 degrees or less (discarded); drive-thru soda nozzle and lobby soda nozzles found with accumulated black debris; slide-thru trays and wire baskets found with built-up grease.

Uncle Herbie’s — 2206 N. 25th St. (2 Critical, 3 Non-Critical) No consumer advisory on menu for raw or undercooked items; inside of ice machine found with black debris.

Wendy’s #77 — 2803 Wabash Ave. (2 Critical, 2 Non-Critical) No proof of certified food handler; observed several flies throughout establishment.

McDonald’s — 3606 U.S. 41 S. (2 Critical, 2 Non-Critical) West side lobby nozzles and drive thru soda nozzle found with black debris; handwash sink found dirty.

Jiffy Mini Mart #531 —  1131 N. Third St. (2 Critical) Handwash sink observed with mold and debris; hot dogs not cooked to proper temperature (135 degrees) within two hours.

7th & Elm Bar and Grille —  729 N. Seventh St. (1 Critical, 3 Non-Critical) Certified Food Handler expired, no proof of renewal.
