Los Tres Caminos, 5147 S. U.S. 41, (2 Critical, 3 Non-Critical) – Observed several different raw and cooked food in walk-in cooler at 50-51 degrees should be 41 degrees or less; chile rellenos not marked with date of consumption.

Wade’s Short Stop, 7269 State Road 46, (1 Critical, 2 Non-Critical) – Employee observed assembling sandwiches with bare hands.

Four Sons Family Restaurant, 320 S. Third St., (1 Critical, 2 Non-Critical) – Hard boiled eggs not marked with date of consumption.

Terre Haute Bowling Center, 600 E. Springhill Drive, (1 Critical, 2 Non-Critical) – Quik and Crispy oven basket found with accumulated soil and grease.

Illiana Lounge, 925 National Ave., (1 Critical, 1 Non-Critical) – Observed black debris inside of ice machine.

Zorah Shriners, 420 N. Seventh St., (1 Critical, 1 Non-Critical) – Spray bottle containing toxic substance not labeled found on counter in kitchen.

Subway, 3206 Wabash Ave., (1 Critical, 0 Non-Critical) – Spray bottle of oven cleaner hanging on rack over prep table with open employee drinks.

Sarah’s Philippines Oriental Store, 114 N. 13th St., (1 Critical, 0 Non-Critical) – Packaged dried fish held at room temperature – discarded.

Smoothie Hut Inc., 3401 S. U.S. 41, (1 Critical, 0 Non-Critical) – Yogurt smoothie mix found at 63 degrees should be 41 degrees or below.
