European Grill, 2949 S. 5th St., (4 Critical, 5 Non-Critical) – Observed black debris on soda fountain nozzles; Found baked potatoes not marked with date of consumption; Mashed potatoes, lasagna, and stuffed mushroom past date of consumption; Observed sliced mushrooms, parmesan cheese, stuffed mushrooms and baked potatoes at 55-83 degrees, should be 41 degrees or below.

Taco Bell #2085, 3132 E. Wabash Ave., (2 Critical, 2 Non-Critical) – Cooked potatoes not marked with time to discard; Observed black debris on drive thru soda nozzles and pink debris on spray nozzle at 3-bay sink.

Spring Mill Bread Co., 78 W. Honey Creek Pkwy., (2 Critical, 1 Non-Critical) – Employees observed wrapping muffins with bare hands; Bread pans observed with accumulation of burnt debris.

Uncle Herbie’s, 2206 N. 25th St., (2 Critical, 1 Non-Critical) – Found cooked potatoes and hash browns at 78-90 degrees, should be 135 degrees or above; Found sausage, cooked ham and raw eggs at 65-77 degrees, should be 41 degrees or below.

Ole Simrell’s, 804 S. 7th St., (2 Critical, 1 Non-Critical) – Chili in walk-in cooler held beyond date to discard; Mold on chute inside ice machine.

Long John Silver’s #47, 2039 Lafayette, (2 Critical, 0 Non-Critical) – Mold observed on chute inside ice machine; Scoop and ruler observed on back of handwash sink.

Quickserve, 2455 Lafayette, (1 Critical, 1 Non-Critical) – Toquitos, sausages and hot dogs on rollers observed at 115-128 degrees, should be 135 degrees or higher (discarded).

Fast Max/Sunoco #143, 2219 Lafayette Ave., (1 Critical, 1 Non-Critical) – Soda fountain nozzles observed with mold.

Antique Dairy Queen, 395 Davis Ave., (1 Critical, 1 Non-Critical) – Observed pink debris inside of ice machine.

Booker T. Washington Park Community Center, 1101 S. 13th St., (1 Critical, 1 Non-Critical) – Large containers of soups in first refrigerator not marked with date of consumption.

Wal-Mart SuperCenter, 2399 S. St. Rd. 46, (1 Critical, 0 Non-Critical) – Shredded and sliced cheese in open face cooler at 50-55 degrees, should be 41 degrees or below.
