TERRE HAUTE — June 25-29:

Rick’s Smokehouse & Grill, 3401 S. U.S. 41, (3 Critical). Handwash sink blocked by linens basket and CO2 cans; potato salad 60 degrees in cooler — should be 41 degrees or less (discarded); potato salad held beyond date of consumption (discarded).

The Landing at Fort Harrison, 3350 N. Fourth St., (2 Critical). Pink debris found on ice machine chute; cooked chicken, rice, baked potatoes and cut melon not marked with date of consumption.

Penn Station No. 194, 2736 E. Wabash Ave., (2 Critical). Stainless steel baking sheets found with built-up debris; fries in cooling process found in deep plastic containers in cooler, not an approved cooling method.

Subway, 423 W. National Ave., West Terre Haute, (2 Critical). In walk-in cooler, packages and containers of ham, turkey, chicken strips, cold cut trio, shredded and sliced cheese, and salami found at 50-62 degrees — should be 41 degrees or less (discarded); observed dirty dishes on top of back handwash sink.

AMS Gas & Convenience Inc., 301 S. Third St., (1 Critical, 1 Non-Critical). Soda fountain nozzles found with excessive build up of black debris.

Stables Steakhouse, 939 Poplar St., (1 Critical, 3 Non-Critical). Inside of ice machine found with pink debris.

Zorah Shrine Temple, 420 N. Seventh St., (1 Critical). Opened container of potato salad not date marked — manufacturer’s date expired — must discard.

Pilot Travel Center No. 297, 5555 E. Margaret Ave., (1 Non-Critical).
