RESTAURANT INSPECTIONS: Nov. 21, 2011

TERRE HAUTE — The Vigo County Health Department inspected the following establishments Oct. 31-Nov. 4.

Denny’s Restaurant, 222 S. Third St., (4 Critical, 4 Non-Critical). Soda fountain nozzles found with black debris; found dirty tongs hanging on clean rack in dishroom; waffle press/iron found with built-up debris; hash brown in under counter cooler found at 50 degrees — should be 41 degrees or less.

Tokyo Japan, 3401 S. U.S. 41, (4 Critical, 3 Non-Critical). Observed roaches in/on grill/stove in kitchen; found black debris inside ice machine; Maki chicken, eggrolls and home fries not marked with time to discard; cover to floor drain under three-bay sink missing, establishment using colander in drain to catch debris.

Valley Grill, 2170 N. Third St., (4 Critical, 3 Non-Critical). Vegetable soup and chicken noodle found at 127 degrees and 134 degrees, respectively — should be 135 degrees above; cooked spaghetti noodles found at 73 degrees in walk-in cooler after six hours of cooling (discarded) — should be 41 degrees or less; corn beef hash, ham, pulled pork, spaghetti sauce, corn, beef roast, potatoes, noodles not date marked; no sanitizer levels measured in warewashing machine.

Cajun Café, 3401 S. U.S. 41, (4 Critical, 2 Non-Critical). Observed black debris inside ice machine; potatoes, bourbon chicken and eggrolls not marked with time to discard; barbecued chicken, Cajun chicken and blackened chicken found at 116-120 degrees — should be 135 degrees or above; found dead gnats in bucket of bourbon sauce (discarded).

Penn Station East Coast Subs 182, 3642 S. U.S. 41, (3 Critical, 1 Non-Critical). Blanched fries in cooler 63 degrees after two hours; blanched fries time marked to be held past four hours; hot water heater not working.

Taj-Mahal, 1349 S. Third St. (3 Critical, 1 Non-Critical). No Certified Food Handler on staff; Veggie Samosa and Gobi Pakara on hot food bar at 102-108 degrees — should be 135 degrees or higher; no sanitizer dispensing in automatic dishwasher.

Park Ave. Diner, 2967 E. Park Ave., (3 Critical). Potato soup and tomato soup held past date of consumption (discarded); hard-boiled eggs and baked potatoes not marked with date of consumption; no proof of Certified Food Handler.

Burger King 12490, 4790 S. U.S. 41, (2 Critical, 1 Non-Critical). Lobby soda nozzles found with black debris; mushrooms found at 83 degrees — should be 135 degrees or above (discarded).

Illiana Lounge & Pizzeria, 925 W. National Ave., (2 Critical, 1 Non-Critical). No sanitizer measured in automatic dishwasher; sausage, pepperoni and raw eggs found at 51-52 degrees, should be 41 degrees or less.

Magic Wok, 3401 S. U.S. 41, (1 Critical, 4 Non-Critical). Inside of ice machine observed with mold, fry strainer observed with food debris after cleaned and stored.

Little Caesars, 2520 Wabash Ave., Unit N, (1 Critical, 3 Non-Critical). Stainless steel sheet pans found with greasy buildup.

Citgo Food Mart, 743 Lafayette Ave., (1 Critical, 2 Non-Critical). Handwash sink dirty.

Circle K 2255, 1280 Lafayette Ave., (1 Critical, 1 Non-Critical). Working spray bottle of cleaner not labeled.

Vigo County Jail, Canteen Services Inc., 201 Cherry St., (1 Critical, 1 Non-Critical). Stock pot of chicken broth not date marked.

Lone Star Steakhouse & Saloon, 3060 S. U.S. 41, (1 Critical). Food debris on cleaned ice cream scoop and plastic food containers.

Moon Dawg Lounge, 408 W. National Ave., West Terre Haute, (1 Critical). No Certified Food Handler on staff.

The Pit Stop, 6321 N. Clinton St., (1 Critical). Cooked chicken wings held past date to discard.

Bohannon’s East, 1728 Wabash Ave. (2 Non-Critical).

Ichiban Japanese Steakhouse and Sushi Bar, 3495 S. Fourth St., (2 Non-Critical).

Blondie’s Cookies, 3401 S. U.S. 41, (1 Non-Critical).

Jimmy John’s, 3734 S. U.S. 41, (1 Non-Critical).
